• For MS thesis defense:
  • If passed qualification exam at the Master/PhD level: Master's Comprehensive Exam Verification
  • Otherwise, a Master's thesis is needed:
    • Report of Examination Result
    • Master's Thesis Signature and Approval Form
• For PhD proposal defense:
  • Report of Examination Result
  • Doctoral Committee Appointment Request
  • Doctoral Comprehensive Exam Verification
• For PhD dissertation defense:
  • Report of Examination Result
  • Dissertation Signature and Approval Form
• For PhD z-status application:
  • Request for Special Enrollment
  • Reduced Course Load Request For F-1 and J-1 Student Visas

Note: The Report of Examination Result form is filled out for the department's records. The Reduce Course Load Request form should be sent to the International Student Services. All other forms should be sent to the Graduate School.
